L’hermitte's (Electric shock sensation radiating down spine with neck flexion)

L’hermitte's symptom is the occurrence of episodic tingling and electric current-like sensations down the back, into the arms and even down the legs. It is brought on by forward neck flexion. It occurs in 30-40% of all patients in the course of the disease and while not uniquely specific to MS, it is most commonly associated with it. It is usually self-limited but in rare cases, where it may be disabling due to its frequency, it usually responds to carbemazepine therapy.

Notes: Can also be brought on by lateral neck flexion, flexion of dorsal spine, or sudden jolt. Rarely persists.

Symptom Management:
If bothersome, this can be helped with carbemazepine, starting low and building dose gradually until desired relief is achieved.
